Eligibility

Children may be eligible for these services if they:

  • Are infants or children with chronic, complex, or medically fragile conditions
  • Require skilled nursing, therapy, or medical monitoring at home
  • Are under the care of a licensed physician
  • Have a documented plan of care
  • Can safely receive services in the home environment
  • Meet MDH and payer medical necessity requirements
