Eligibility

Individuals may be eligible for live-in care if they:

  • Require 24-hour supervision or care due to medical, cognitive, or functional needs
  • Are appropriate for in-home care and can safely reside at home
  • Have a documented plan of care developed by a licensed provider
  • Meet MDH and payer criteria for continuous care
  • For minors, have parent or legal guardian consent
